By clicking the "Submit Application" button, the applicant(s) specifically
represents to said agency and to agency's actual or potential agents the information
provided in this application is true and correct as of the date set forth by "electronic
signature" via the "Submit Application" button. I understand that
this is not an offer to refinance my existing mortgage loan. My transmission of
this application as an "electronic record" containing my "electronic
signature," as those terms are defined in applicable federal and/or state laws
(excluding audio and video recordings), or my electronic internet transmission of
this application containing my "electronic signature", shall be effective,
valid as if a paper version of this application were delivered containing my original
written signature. I (we) hereby authorize United First Financial, it's affiliates,
succors and/or assigns to verify my past and present employment earnings records,
bank accounts, stock holdings, and any other asset balances that are needed to process
my MMA account application. I further authorize said agency, it's affiliates, succors
and/or assigns to order a consumer credit report and verify other credit information,
including past and present mortgage and landlord references. It is understood that
a copy of this form (application) will also serve as authorization.
